Registered Nurse - RN/LPN

Position Open.

General Summary: An non-exempt hourly position responsible for providing primary nursing care to patients including assessment, treatment, patient education, and clinical support to medical office practioners within scope outlined by the Vermont State Board of Nursing and Vermont Statutes.

Essential Job Responsibilities:

1. Obtains patient consent for care and ensures patient confidentiality. Obtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition. Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.

2. Collaborates with our team to meet requirements of the Patient Centered Medical Home and the Accountable Care Organization.

3. Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response. Delegates as allowed under the Nurse Practice Act. Refers to and consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.

4. Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices in electronic health records.

5. Participates in multidisciplinary teams to improve patient care and processes and outcomes. Tracks quality assurance data and monitors for acute and chronic care management.

6. Medication refills per physician protocols

7. Schedules tests and appointments with appropriate prior authorization requirements.

8. Triage patients by phone and at the practice site per Tamarack policies and procedures, provide appropriate medical advice to patients and consult with a practioner as appropriate.

9. Instruct and prepare patient, assemble equipment and assist provider for procedures and routine exams as appropriate.

10. Participate in and manage, as appropriate, communication with other providers and organizations regarding patient orders and/or directives.

11. Provide injections, medication administration and intravenous therapy as ordered utilizing the five rights of medication administration.

12. Maintain necessary skills and knowledge to obtain and maintain integrity in the collection, storage, packaging, labeling, and transportation of specimens to the lab, following Universal Precaution guidelines

13. Follow CLIA guidelines for laboratory management

14. Clean examination/procedure rooms, bathrooms and equipment as appropriate and necessary.

15. Wash, package equipment for sterilization and store instruments

16. Assist administrative staff as needed and able with scheduling patients, answering telephones and medical record management

17. Assist with new employee training and on-going training for staff

Education: RN or LPN. Completion of nursing training from an accredited school of nursing.

Experience: Clinic experience preferred, but will provide on-site training.

1. Skill in time management, problem solving, multitasking, prioritizing, and medical care coordination.

2. Skill in initiating appropriate crisis interventions and emergency response.

3. Skill in patient triage in person and on the phone.

Abilities:

1. Ability to analyze options and then counsel patients and families about choices and make referrals to other providers and resources.

2. Ability to engage clients and families in the development and implementation of a care plan.

3. Ability to read and interpret physicians’ orders and notes from other providers.

4. Ability to calculate and administer drug dosages and injections and measure results.

Equipment Operated: Standard nursing equipment and supplies including thermometer, syringes, blood pressure machine, nebulizer, glucometer, Hgb machine, EKG machine, lab procedures such as strep test,

urinalysis and pregnancy tests. May involve use of assistive devices such as walkers, wheelchairs, ventilators, pulse oximetry units, and oxygen tanks. Will require use of computer and electronic health record.

Work Environment: Medical exam/treatment rooms and medical offices. Controlled lighting, acoustics, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.

Mental/Physical Requirements: Involves standing, walking, bending, grasping, manipulating, and squatting. Work may include occasional pushing, pulling, or carrying objects weighing approximately 50 pounds such as files, documents, equipment and supplies.

Front Desk Receptionist/Scheduler/Medical Records

Position Filled.

General Summary:  A nonexempt hourly position responsible for performing a variety of clerical duties for office manager, clinical services manager, or physicians.

Essential Job Responsibilities: 

1.       and transferring phone calls.

2.       Obtains, verifies, and updates patient information and provides support services to patients and medical staff.

3.       Checks patients into electronic scheduling system upon check in and schedules appointments as needed when patients leave.

4.       Collects patient copayment, deductible or office visit fees as required.

5.       Assists in obtaining eligibility from patients’ insurance companies for their upcoming appointment.

6.       Assists in obtaining any necessary prior authorization required from patients’ insurance companies.

7.       Copies and faxes materials as necessary.

8.       Obtains and processes incoming and outgoing mail and faxes daily

9.       Properly distributes and indexes received electronic faxes.

10.   Assists in care and maintenance of office equipment.

11.   Sorts and distributes daily mail to appropriate departments and staff.

12.   Maintains the patient EMR and document placement within the EMR

13.   Scans and data enters results for colonoscopies, stress tests, diabetic eye exams, etc in EMR

14.   Performs medical records requests on behalf of patients, with signed authorizations, indexes medical records to appropriate chart upon receipt.

15.   Ensure appropriate privacy and confidentiality documentation is initiated and/or maintained for each patient.

16.   Coordinate with other providers or facilities for additional treatment or procedures as necessary.

17.   Ensure accuracy of all patient financial transactions including daily balancing of cash drawers, credit card reports and maintaining daily cash log as necessary

18.   Maintain clean and orderly reception, waiting room, counter areas, customer restroom area and entryway.

Education:  High school diploma or general equivalency diploma (GED).  Diploma as a medical administrative assistant from an accredited medical vocational course preferred. 

Experience:  Two years of front desk, reception and/or customer service experience in a medical office setting preferred.  Excellent personal and interpersonal skills required. Excellent customer service skills required both in person and over the telephone

Performance Requirements: 

Knowledge: 

1.       Knowledge of medical terminology and office procedures.

2.       Knowledge of basic arithmetic to make calculations, balance and reconcile figures, and make changes accurately.

Skills:

1.       Skill in operating office equipment.

2.       Skill in organizational matters, including time management, prioritization, multitasking, and problem solving. 

3.       Excellent customer service skills required both in person and over the phone

Abilities:

1.       Ability to sort and file materials correctly using alphabetic or numeric systems.

2.       Ability to communicate clearly and concisely.

3.       Ability to establish and maintain effective working relationships with patients, physicians and other clinical staff, and the public.

 

Equipment Operated:  Office machinery including computers, fax machines, calculator, and photocopier.

Work Environment:  Work performed in office environment.  Involves frequent contact with staff and patients.

Mental/Physical Requirements:  Manual dexterity for office machine operation including computer and calculator, stooping, bending to handle files and supplies, mobility to complete errands or deliveries, or sitting for extended periods of time. Stress can be triggered by multiple staff demands and deadlines.  Work may include occasional pushing, pulling, or carrying objects weighing approximately 50 pounds such as files, documents, equipment, etc.
