Tamarack Family Medicine is a Patient Centered Medical Home

To all our patients: 

Mission: 

We are a medical home committed to creating an environment where health care is accessible, family centered, continuous, comprehensive, coordinated, compassionate, and culturally competent.  We are concerned about your health and aim to ensure whole person care including preventative care, chronic care management and behavioral health care.  We view ourselves as care partners with you, working to develop comprehensive care plans, promote self-management, and offer community- based support to meet your health care needs.    

As a new patient, you will be assigned to a primary care provider who will work closely with you to optimize your health.  Provider assignment is based on patient choice and availability of the provider.  Every effort is made to accommodate requests. 

As your medical home we will: 

  • Take care of your health care needs when you are sick and help you stay well 

  • Plan your care with you and set goals for care, now and in the future 

  • Talk with you about any testing and treatments that you may need 

  • Work with you and other care providers to coordinate care 

  • Provide resources, education and information regarding medications, specialists, community health team resources, home care, equipment, support services for you and your family, and other key local services as needs arise. 

 

Services provided:  

  • Direct medical care, including care for chronic problems, acute care, and preventative care.  

  • Minor surgical procedures skin procedures, joint injections, wound care, placement of long acting birth control, and vasectomy 

  • Prescription refills 

  • Vaccines provided by the Vermont Health Department, as well as seasonal flu shots 

  • Coordination of referrals to specialists or other health care providers. If you prefer to schedule your own appointments we support that as well.  We only request that you keep us updated when these appointments occur so we can be sure to track and review the reports and notes. 

 

Office Hours and Phone (802)851-0999 

  • Office hours are Monday through Friday: 8:00 am to 4:30 pm, Saturday: 9:00 am to 11:00 am for acute and routine visits upon request. 

  • TFM providers are available to you 24/7. If you need medical advice outside of normal business hours, please call the office and the answering service will page the on- call provider. Your calls will be returned within 30 minutes unless the provider is involved in an emergency, if that is the case, they will return your call as soon as they are available. 

  • For patients needing medical advice during normal business hours, we are committed to returning call within 24 hours but most likely it will be by the end of the business day. 

  • For patients needing to be seen urgently, we provide same day appointments, routine physicals are scheduled with 90 days of request, post hospitalizations follow up appointments are scheduled within 7-14 days. Our staff work hard to meet your special requests so please let us know what your needs are. 

  • Video or telephonic appointments may be available upon request. 

 

What can you do to help meet your treatment goals? 

  • Be prepared to share how you are doing and any particular issues you may be having 

  • Share past medical history about yourself and self-care information 

  • Write down and bring in your questions and concerns 

  • Bring a list of other providers you may have seen since you last visit 

  • Be an active participant in planning your care and self-management goals 

  • Bring your insurance card and co-pay if you have one 

  • Bring any forms you would like us to complete 

  • Bring a detailed medication list or even bring the bottles in with you
